Dog Day at the Park

On Friday, June 28, the A's will hold their 8th Annual Dog Day at The Park at the O.co Coliseum, presented by AvoDerm, Nylabone & Bio Spot. Fans are invited to bring their dogs to the park and help cheer on their home team as the A's host the St. Louis Cardinals at 7:05 p.m. This year's event will feature a pregame Pup Rally and Pup Parade around the field.

The fun begins at 5:20 p.m. with the Pup Rally, where each pup will receive a goodie bag from AvoDerm, Nylabone & Bio Spot. Immediately following the Pup Rally, fans will parade around the warning track of the field with their dogs.

The A's will donate a portion of the proceeds to Dogs4Diabetics and the Animal Rescue Foundation (ARF). Dogs4Diabeticsis a non-profit organization that trains medical-alert service dogs to detect hypoglycemia (low blood glucose) in diabetic youths and adults and alert their partner or other family member of the situation. ARF saves dogs and cats who have run out of time at local shelters. Their vision for ARF is an organization that can not only aid abandoned and homeless animals, but also promote the concept that people's lives can be enhanced by strengthening the bonds between humans and animals.

All dog owners must bring a copy of a current vaccination document from a licensed veterinarian. All dogs must wear an identification tag to the game and be accompanied by someone 18 years of age or older. Online purchase and registration is required.

A portion of the proceeds from the A's Dog Day will benefit Dogs4Diabetics & the Animal Rescue Foundation (ARF)..

The Oakland A's reserve the right to deny any application.
If registering more than four (4) dogs, please return to this form and complete with additional dogs, then buy all of the human and dog game tickets in one transaction. Remember, only one (1) dog per person is permitted.

	I, the undersigned, fully understand, appreciate and agree that I have voluntarily requested to participate in an event entitled "Dog Day" at the Coliseum in Oakland.  "Dog Day" will involve my attendance and the attendance of one (1) dog of which I have custody.  I understand that by participating in this event, my actions may result in anticipated and/or unanticipated injuries and damage to myself and others including my dog; as well as the dogs of others.  I hereby represent that the dog of which I have custody during the event is in good health.  I confirm and assure that the dog has current rabies vaccinations.  I agree to abide by any decision of any Athletics official, or third party contracted on behalf of the Athletics, concerning my ability or the ability of the dog of which I have custody to participate in this event safely, and I further agree that Athletics officials may authorize necessary treatment for me and/or the dog in my custody.  I understand, appreciate and agree that I assume any and all the risks associated with my participation in this event including, but not limited to, injury and property damage including injury to dogs, illness, falls, contact with spectators at the event, injuries or damages sustained by and through other participants' dogs, the effects of the weather including extreme temperatures and humidity, the condition of the Coliseum in Oakland, including all sidewalks and parking lots.  I agree to abide by all the rules and instructions given in connection with this event.  I assume the risk for injuries and damages sustained as a result of the manner and method by which the event is administered.  I acknowledge that I have completed the "Dog Day 2013 Application."  I understand that I am solely responsible for the accuracy of information that is contained therein, and I hold the parties as identified below released from liability and any responsibility for verifying the information contained in the application whether the information was provided by me or a third party.

	I, the undersigned, have read this waiver, am at least 18 years old, and I am competent to sign the waiver with full understanding of the above information.  In consideration for allowing me to participate in this event with the dog in my custody, I hereby for myself, my heirs, executors, and administrators or anyone acting on my behalf, waive, release, and covenant not to sue and fully indemnify, hold harmless and discharge the Oakland A's, Athletics Investment Group LLC, Oakland-Alameda County Coliseum, Inc., Oakland-Alameda County Coliseum Authority, Spectacor Management Group, the City of Oakland, and their respective affiliates, predecessors and successors, owners, agents, partners, officials, employees and representatives, any sponsor of "Dog Day," supplier, agents, and independent contractors or other personnel in any way assisting or connected with this event from any and all claims, demands, or causes of action of any kind arising out of my participation in this event, regardless of who causes the injury or damage sustained and regardless of whether based on fault or negligence or not.

	I hereby grant to such parties named in this waiver the perpetual right to use for publicity or promotional purpose, my name or pictures of me and my dog in any media without any liability or obligation to me.

I fully understand, appreciate and agree with the above information.
